Key Request e-Form

Complete the following e-form and submit it electronically. Physics personnel will then prepare the typed University key form and gather the required signatures for you. The signed and approved copy of the typed form will be available for pickup in Weniger Hall 301 (the Physics main office) within about 48 hours. Take the approved typed form to the University Key Shop at 510 SW 15th Street . There is a $5 deposit required for each key; this is refundable upon return of key to the University Key Shop. You are required to pay the deposit when you get your keys. A photo ID is required and the official University approved typed form must be signed by authorized Physics personnel. If you need to return a key for any reason, please call the University Key Shop at 737-3565 between 8:00 am and 4:30 pm to arrange a time to make the change. If you are requesting keys to rooms in more than one building, please submit a separate e-form for each building.
